Medical records are kept for a period of seven years. If you need copies of your student medical records forwarded to another location or sent to us, you will need to grant permission to release/receive your records (PDF). You may also come in during business hours to sign the records release form. You will need to specify the name, address, and fax number for the individual or office the information will be sent to.
